Cervical screening programme – ceasing

Use this service to permanently remove yourself from the NHS cervical screening programme (NHSCSP) due to ineligibility.

You can use this service if you:

• are registered at the surgery

Your responses will be checked to ensure you are correctly ceased. Once you have been removed from the screening list, you will no longer receive correspondence from the screening programme except a letter to confirm removal (excluding radiotherapy to the cervix).

Before you start

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
